For anyone not registering for the online portal, you may download the full version of the New Patient Welcome Packet. Please print and complete it prior to you first office visit.
Condensed Version
For patients with online portal access, you will need to complete the condensed welcome packet with the appropriate clinical forms. Please complete it prior to you first office visit.
*As of August 21, 2024, Painlogics is migrating to online forms for the administrative section of the New Patient Welcome Packet. You can access this section after registering for the Athena online portal, and using the self check-in function. You will need to complete the demographics, HIPAA privacy, financial policy & insurance agreement, medical records release, social history, and the pain agreement forms. For the remaining clinical section of the New Patient Welcome Packet, please download and complete the condensed registration packet.
